Program Specialist (Patient Safety)

The primary purpose of this position is to assure the Patient Safety Program is in compliance with NCPS policies are found in the VHA National Patient Safety Improvement Handbook (directive), national patient safety training materials and computer systems, and applicable regulations of the Joint Commission on Accreditation of Healthcare Organizations (TJC).The incumbent functions as the manager, coordinator, and point of contact for patient safety policies and activities developed by the National Center for Patient Safety (NCPS) at the health care facility to which he/she is assigned. Duties include, but are not limited to:
The incumbent will be the facility's key person to help build and implement patient safety systems and promote the culture of safety within VA facilities and for VA patients in all healthcare settings. Performs duties directly contributing to the administrative management of the facility health care delivery system, and is responsible for planning, organizing, developing, managing, and evaluating the medical center's patient safety program. These duties require the ability to apply the specialized principles and practices of health care management in leading the facility's patient safety program. Performs Root Cause Analysis activities such as:
SAC classification Provides RCA just in time training. Identifies RCA Team members. Supports RCA Teams, e.g., RCA Advisor. Develops competencies in team functions. Ensures that RCAs are complete, that all technical requirements are met, and all implementation requirements are accurately identified. Familiar with and proficient in the use of SPOT, VHA's national patient safety software application. Supports aggregate review process; functions in the role of advisor. Follows-up on RCA Actions/Outcome Measures. Resident expert on confidentiality rules as they apply to the patient safety process. Works with, trains, and directs patient safety activities of similar personnel within the facility, such as risk managers and performance improvement. Responsible for translating the policies, theories, and strategies into action within the complex interrelationships that exist related to patient safety. Responsible for training and implementation of the patient safety, RCA, and HFMEA process to clinicians, management, and all relevant support personnel within their facilities and associated facilities and operations (e.g., home care). Performs patient safety programmatic functions including:
Serves as the facility point of contact for Patient Safety Alerts/Advisories, including tracking system actions. Serves as member of various medical center committees, as appropriate to the position, e.g., Safety/Environment of Care. Conducts periodic environmental patient safety inspections. Collaborates with Biomedical Engineer, Occupational Safety Manager, Industrial Hygienist/ Infection Control, etc. Shows leadership in publicizing and promoting patient safety successes and activities, e.g., town meetings, poster sessions, news flashes, awards, etc. Disseminates Patient Safety Reporting System (PSRS) program information. Provides new employee patient safety orientation and training. Develops facility patient safety training program. Acts as the Point of Contact for facility patient safety issues with NCPS. Serves as the facility expert on the VHA Patient Safety Improvement Handbook. Initiates and advises on Healthcare Failure Mode Effects Analysis (HFMEA) evaluations. Familiar with and addresses results of periodic safety culture survey with action plan for facility. Acts as liaison on patient safety issues. Plays a critical support role in developing and coordinating internal review systems to assure that both clinical and administrative patient safety activities are in compliance with agency and accrediting standards and regulatory requirements, including:
VHA National Center for Patient Safety JCAHO Patient Safety Standards VHA Patient Safety Performance Measures External agency reporting requirements for FDA, ORCA, SMDA. Analyzes data, identifies trends and concerns of issues. Brings to the attention of the Medical Center Director those areas where possible discrepancies exist. Maintains and monitors program requirements/validation criteria for the facility's patient safety program. Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. Experience:
You must have one year of specialized experience equivalent to at least the next lower grade GS-09 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to:
Analyzing data and identifying trends; Preparing and/or reviewing reports involving trend analyses; Developing a patient safety training program; Translating policies, theories, and strategies related to patient safety; Utilization of information management systems to support patient safety analysis techniques Communicating effectively to gather information, present recommendations, and coordinate service All qualifying experience you possess must be clearly described in your application package.
