Social Worker (Program Coordinator)

The social worker in this position will report to the Health Care for Homeless Veterans Program Manager, in the Patient Care Service Line at the Iowa City VA Health Care System. The VJO/HCRV Case Manager provides services under the direction of the HCHV Homeless Program Manager. The clinical social worker assigned to this position will provide outreach services to a highly complex and high-risk population detained in secured, county, state, and federal correctional institutions The clinical social worker's VJO responsibilities are conducted primarily in community settings in conjunction with police encounters and correctional facilities, including county jails, work release centers, and local courts. The program coordination responsibilities include developing the local program. This position is separate from the main facility and supervisors, requiring an advanced level of practice and the ability to work independently. This position requires mastery of a wide range of qualitative and/or quantitative methods for the assessment and improvement of program effectiveness and advocating for the special needs of the population, in addition to knowledge of the principles and theoretical concepts of Social Work. The incarcerated and ex-offender population present with health care and psychosocial problems that are complex and require a high degree of clinical oversight and creative problem-solving. The Healthcare Reentry duties include outreach to Veteran's incarcerated in state and federal prisons in the states of Illinois and Iowa, within the Iowa City VA Medical Center's catchment area. The Case Manager provides assistance to the Veteran in determining eligibility and accessing services at the time of release (usually 3 to 6 mos.) to provide for a transition from incarceration to the community. The VJO/HCRV Case Manager will manage a full clinical workload, including psychosocial/diagnostic assessment with patient/family. The Case Manager demonstrates ability to establish therapeutic rapport, demonstrates effective interviewing skills, and elicits relevant information by case presentation and observation. The Case Manager initiates outreach to justice involved Veterans. The target population consists of Veterans who are involved with the criminal justice system or are incarcerated in state or federal correctional facilities. Provides coordination of outreach services with county correctional institutions and collaboration with the justice system, including classification officers, probation officers, re-entry staff, public defenders, and private attorneys. The Case Manager provides therapeutic interventions that are consistent with the psychosocial assessment. Professional services include, but are not limited to, evaluation and assessment, supportive counseling, crisis counseling, treatment planning, patient and family education, case management/care coordination involving VA and non-VA community resources. Incumbent demonstrates the ability to conceptualize, communicate, prescribe, and provide intervention to patient and/or family. The Case Manager ensures continuity of patient care in the development and implementation of the plan of care. Effectively communicates with and utilizes other disciplines (e.g., nursing, medicine, psychology) to facilitate treatment planning and implementation. Participates effectively in team meetings, treatment planning conferences, etc. Collaborates with divergent multidisciplinary team members in a manner that enhances coordination of comprehensive patient care. The Case Manager provides or facilitates professional education to law enforcement and other correctional facilities concerning unique Veteran issues. The Case Manager provides liaison with Veterans Courts, Drug Courts, or Mental Health Courts. The Case Manger will assist in facilitating the creation of any of the above courts with community partners. Facilitates evaluation and treatment of Veterans. The Case Manager provides all necessary direct and indirect patient care services consistent with area of assignment. The incumbent demonstrates the ability to balance quantity and quality requirements. In most instances, agreed upon priorities are maintained and caseload is effectively managed while keeping the Homeless Program Manager informed of problem areas. The Case Manager demonstrates an ability to perform quality professional psychosocial services for the young, middle age, and geriatric age patients. The Case Manager assists with the CHALENG and Stand Down meetings. Incumbent will need to travel and may have irregular tours of duty. Incumbent may be expected to provide appropriate clinical backup and guidance to social work staff in the mental health service line. Work Schedule:

Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. Social Worker (Program Coordinator), GS-12 (1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. REMINDER:
