Qualifications:
Bachelor's degree in education, social work, counseling, communications, or other related degree Ability to communicate effectively and professionally in English and Spanish (French may be considered) Demonstrated ability to collaborate with other professionals Demonstrated ability to build rapport with parents and children of all ages.
Understanding of the underlying concerns that may cause youth to be identified as at-risk within the educational environment, including but not limited to:
barriers to accessing healthcare parent engagement poverty and homelessness mental health Computer skills in word processing, developing spreadsheets and tables, and navigating the internet Demonstrated ability to speak fluent Spanish, in person and on the phone, with written Spanish a plus Initiative to be a self-starter and complete tasks unsupervised Excellent organizational skills and ability to handle several tasks simultaneously Knowledgeable about community health care and other resources preferred but not required Experience in health care and/or non-profit preferred but not required Such alternatives to the above qualifications as the District may find appropriate and acceptable.

Health Services Manager (in collaboration with Healthy Kids School Based Clinic Coordinator) SUPERVISES:
NA JOB GOALS:
To assist with coordination of the day-to-day functioning of the Healthy Kids school-based health clinics, and provide health care case management to students and families, in order to enhance student learning, promote the overall efficiency of the school system, and maximize the educational opportunities available to each child.
PERFORMANCE
Responsibilities:
Provide information to families and answer questions regarding school-based clinic services, including mental health services and dental services.
Schedule patients for assigned clinic sessions and send appointment reminders in a timely manner.
Check-in/out patients using the clinic electronic record system during assigned clinics.
Set up equipment and prep supplies for assigned clinic days Obtain parental consent for clinic services and assist parents in filling out paperwork, if needed Provide interpreting services and translation of clinic materials as needed Greet and escort students/parents to exam rooms and assist with taking/recording vitals, if needed Ensure that clinic sites maintain compliance with OSHA, CLIA, IRIS, HIPAA, and other agencies or programs as required.
Work in conjunction with healthcare providers to relay messages and act as a liaison for ongoing care management and needs to clinic parents and patients.
Must be comfortable handling medically sensitive messages.
Provide case management to students and families receiving long-term care from the clinic.
Troubleshoot and connect families to community resources Arrange transportation for families to and from appointments at the clinic or partner clinics.
Ability to be mobile and provide your own vehicle for transportation to and from clinic locations.
Work collaboratively with district support staff - Student & Family advocates, school nurses, and athletic trainers - to set up appointments and follow up on care management for patients.
Work collaboratively with area agencies to provide ongoing care management to short and long-term clinic patients.
Make referrals, assist with scheduling, and follow up on patients receiving outside care (eye exams, dental appointments, mental health care, reproductive health care, etc.
).
Work with Clinic Coordinator and clinic healthcare providers to maintain the clinic supplies.
Work collaboratively with Clinic Coordinator and other clinic staff to maintain accurate record-keeping of patient data.
Maintain compliance with the Vaccine for Children program - confirm patient eligibility, precise recordkeeping, proper storage, and handling, etc.
Assist with managing the clinic's social media platforms.
Participate in community outreach events as needed.
In conjunction with the Clinic Coordinator, assist with developing and facilitating parent/patient advisory councils or focus groups as directed.
Other duties as assigned.
TERMS OF EMPLOYMENT:
20 hours/week M-F - Temporary one-year grant-funded position.
Hours may vary from 8-5 pm based on the clinic's needs.
Hourly wage of $25/hour.
EVALUATION:
Performance of this job will be evaluated in accordance with provisions of the District's policy on Evaluation, State Statute, and any applicable Collective Bargaining Agreement.

Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
